I'd suggest "Inviting Disaster"[1] as an expansion on this topic.  Lots of
interesting stuff on "Incidents" and the few small seeds that lead to them.

(But, in the worlds that most of us live in, there are going to be corners
cut and the redundancy will get "scoped out"...)
