The server responds with the 3 records in a (usually) random order.
It's up to the MTA to figure out preference.

For instance for me, it changes for pretty much every query:

kluge.net. 86400 IN MX 10 mail.bblisa.org.
kluge.net. 86400 IN MX 10 bugzilla.spamassassin.org.
kluge.net. 86400 IN MX 5 eclectic.kluge.net.

kluge.net. 86400 IN MX 10 bugzilla.spamassassin.org.
kluge.net. 86400 IN MX 5 eclectic.kluge.net.
kluge.net. 86400 IN MX 10 mail.bblisa.org.

kluge.net. 86400 IN MX 5 eclectic.kluge.net.
kluge.net. 86400 IN MX 10 mail.bblisa.org.
kluge.net. 86400 IN MX 10 bugzilla.spamassassin.org.

but MTAs know they should take the 5, then either of the 10s.
